About the Program

What is CalAIM?

CalAIM (California Advancing and Innovating Medi-Cal) is a statewide initiative designed to improve health outcomes through a more coordinated, person-centered approach to care.

CalAIM expands the way Medi-Cal supports members by addressing not only medical needs, but also the social and environmental factors that can affect health, safety, and independence.

One component of CalAIM is Community Supports, which allows participating Medi-Cal Managed Care Plans to offer services that may help eligible members remain safely in their homes and communities.

Understanding the Benefit

What Are Community Supports?

Community Supports are optional services offered by participating Medi-Cal Managed Care Plans for eligible members with identified health, functional, or caregiver support needs.

These services are intended to complement medical care and help individuals maintain their independence, reduce avoidable hospitalizations, and support successful living in the community whenever possible.

Community Supports are not available to all Medi-Cal members, and service availability may vary by health plan and individual circumstances.

What We Deliver

What Services May Be Available?

Depending on a member's needs and health plan authorization, Community Supports may include services such as:

Personal Care Assistance

Support with activities of daily living, including:

  • Bathing
  • Dressing
  • Grooming
  • Mobility assistance
  • Transferring and ambulation

Homemaker Services

Assistance with tasks that help maintain a safe home environment, including:

  • Meal preparation
  • Laundry
  • Light housekeeping
  • Shopping and errands

Family Caregiver Support

Temporary relief and support that may help family caregivers continue providing care safely and effectively.

Transition Support

Services that may assist eligible members who are returning home following a hospitalization, rehabilitation stay, or other care transition.

Services vary based on health plan benefits, member needs, and authorization requirements.

Eligibility

Who May Be Eligible?

Community Supports may be considered for individuals who:

  • Need assistance with daily activities
  • Have chronic or complex health conditions
  • Are at risk of hospitalization
  • Are at risk of nursing facility placement
  • Need support to safely remain at home
  • Have caregivers who may benefit from additional support services

A member's physician, care manager, hospital discharge planner, social worker, or healthcare team may help identify whether Community Supports should be explored.

Authorization

How Are Services Approved?

Eligibility and authorization decisions are made by the member's Medi-Cal Managed Care Plan.

The health plan evaluates each member's individual circumstances and determines whether Community Supports can be authorized based on program requirements and assessed needs.

Not all Medi-Cal members qualify, and authorization is not guaranteed.

Next Steps

How Can Members Learn More?

Members who believe they may benefit from Community Supports can:

  1. 1Contact their Medi-Cal Managed Care Plan.
  2. 2Speak with their care manager or Enhanced Care Management (ECM) provider.
  3. 3Discuss their needs with their physician or healthcare team.
  4. 4Ask whether Community Supports may be appropriate for their situation.

The member's health plan is responsible for determining eligibility and authorizing services.
